The intent of the Fund is to provide that medical treatment CST deems necessary to return a dog to service.  It is not the intent of the
Fund to supplement fund raising efforts for individual Clients, but to assist in critical and emergency care situations.
The Fund was created for Clients that have no other way to pay for critical and emergency care veterinary expenses.  As such, the
Fund is not available to Clients or families that have alternate methods of payment.

Introduction
CST cannot be held responsible for misdirected or lost applications.  The application may be submitted via Certified U.S. Mail,
facsimile, or e-mail transmission.  If CST staff does not contact you within five working days from documented receipt of sending the
application, contact CST directly.  Please save a copy of this application. It may need to be resubmitted if any problems occur.
Of primary concern to CST is the health and welfare of the dog and that immediate care is provided as soon, as is reasonably
possible.  Therefore, CST staff will contact you as soon as the application has been accepted for processing.  CST staff will notify you
in writing if further information and/or documentation are required.  Unless otherwise stated, this additional information and/or
documentation must be received within five business days of the request or your application will not be accepted.  

Guidelines for Funding
A committee of not less than three people will make Fund decisions.  The committee will consist of Carol Roquemore – CST CEO and
at least one veterinarian in the discipline for which the medical treatment is requested.  
In making Fund decisions, the future abilities of the dog to meet the service requirements of the Client, including the age of the dog,
will be of primary importance in our evaluation.  To assist with the decision making process, eligibility guidelines have been developed.  

THE FUND RESERVES THE RIGHT TO CHANGE, MODIFY OR REMOVE AT ANY TIME WITHOUT NOTICE.

  • Fund eligibility guidelines include the following.
  • Critical care is defined as veterinary surgical medical treatment that is required to resolve an immediate health issue.  
  • Emergency care is defined as veterinary medical treatment that is required to stabilize/diagnose a dog due to a traumatic event.  
  • CST owns the dog (i.e., the dog has not been retired and/or adopted out as a pet).
  • The dog is not terminally ill.
  • It is the opinion of the Fund veterinarian that if medical treatment is received, the dog can return to performing the service dog
    tasks required to meet the needs of the Client.
  • The Fund will not:
  • be used for long-term medical needs
  • pay for unapproved procedures
  • reimburse a Client for procedures where the veterinary clinic has already received payment.
  • Funding is typically for up to 75% of the proposed treatment with a maximum funding set on a case-by-case basis.  All applicants
    must provide assurance that the balance of the medical costs can be paid before the funds are released.  
  • If the situation is deemed an emergency that is life threatening and time is of the essence, Carol Roquemore can release funds
    without committee approval.
  • Funding will only be considered when treatment has been requested pursuant to the following conditions.
  • The applicant provides written documentation from a specialized veterinarian citing the need for treatment and providing an
    opinion regarding the dogs ability to return to service in similar capacity as that which was established prior to treatment
    becoming necessary.
  • The treating veterinarian does not provide any type of payment plan which can include carrying account balances, partial,
    monthly or delayed payments, and/or post-dated checks.  

Qualifications
  • Notification if Client meets the Fund criteria will be made in a timely manner.
  • Due to the volume of Fund requests received, the amount of money each Client can receive in a year must be limited.  On
    an annual basis, the Committee will set the number of approved cases and/or dollar limit.  By not revealing this limit, it is
    the intent of CST to provide at least some funding to those applicants in most need of financial assistance.
  • CST reserves the right to revoke approval of funding for any reason prior to the dog’s treatment.  Reasons for revocation
    include, but are not limited to:
  • unresponsiveness on the part of the Client, and/or
  • determination by CST of falsification or omission of information provided during the application process.
  • CST will investigate all applications, and may obtain verifications from the primary consulting clinic that recommends
    the  treatment and alternate veterinary clinics of proposed treatments and costs.  In all cases, veterinary clinics will
    be asked whether discounts and/or payment plans are available and if the primary clinic has received payment for
    the treatment in question.
  • The Fund committee will approve the treating veterinary clinic prior to commencing with the treatment.
  • Payment will be dispersed from the Fund directly to the treating veterinary clinic.  No additional charges for services,
    treatment or items may be added without Fund committee approval.
  • In the event that other sources of funding are applied for, and/or received, after or during the application process, CST
    must be notified.  If alternate funding is received after payment has been made to the CST approved veterinarian clinic,
    the Fund must be reimbursed.
  • If the Client is unresponsive to their application (e.g., obtains alternate funding, etc.), CST: must receive a copy of all
    medical records, including but not limited to verification of treatment, prognosis, short/long-term medical treatment plan,
    recovery duration, etc., and staff will, within 30-days of Client’s unresponsiveness conduct a home-visit to ascertain the
    health and welfare of the dog.
  • If the dog does/has not received the necessary medical treatment, the dog will be returned to the CST facility.
